- You will be part of TheraTrooper, Inc., an Ensign affiliated company and part of a group of independently operated skilled nursing facilities with long term care, post-acute rehab, and outpatient therapy.
- Effectively and efficiently provided treatment interventions for short-term and long-term rehab and opportunities for outpatient rehab
- Utilizes standardized assessments and evidence-based practice to support clinical interventions
- Administers a wide variety of interventions as part of the plan of care based upon individual patient needs
- Assesses patient’s progress and collaborates with physical therapists on recommendations for adjustments to plan of care per regulatory and clinical practice requirements
- Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders
- Effectively communicates with the OneClinical Team regarding patient progress and barriers
- Collaborates with physical therapists for progress of patient performance, updates to plan of care, and discharge success
