About Pine Crest Health and Memory Care
Pine Crest is an Ensign affiliated facility. Every facility operates with local leadership and autonomy, allowing teams to make decisions that best serve their patients and communities — without unnecessary corporate red tape.
Pine Crest is a 120 Bed Facility, average census 90. Typically have 10-12 skilled patients. We have 2 secured dementia units. Large facility and large therapy department with full kitchen and functional areas within the therapy department including a small mock pharmacy, grocery store, and cafe. Experienced PT, OT and SLP staff of 5 with support from PRN staff. We are proud to be changing the future of post-acute care by dignifying long-term care through clinical excellence, compassion, and innovation.
Qualifications:
Licensed Physical Therapist (PT) in the state of Wisconsin
New graduates welcome to apply
Skilled nursing or post-acute experience preferred, but not required
Strong communication and teamwork skills
Passion for patient-centered rehabilitation
