Belmont Terrace in Bremerton, WA  is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery.  We are currently seeking a part time or full time Occupational Therapy Assistant for our in-house rehab program!
 
Duties:
  • Provide rehabilitative, skilled, and medically necessary treatment interventions to patients with cognitive and/or physical functioning disorders, following regulatory and clinical standards under Occupational Therapist supervision.
  • Administer various rehabilitation procedures, including ADL retraining, therapeutic activities, exercises, modalities, and the use of supportive and assistive devices, under Occupational Therapist supervision.
  • Assess patient progress and communicate with the supervising Occupational Therapist to adjust treatments in accordance with regulatory and clinical practice requirements.
  • Report treatment outcomes’ effectiveness to the supervising Occupational Therapist.
Qualifications:
 Must be a graduate of an accredited Occupational Therapy Assistant program 
Must hold current and active State license/registration where applicable

Duties:
  • Effectively screens/evaluates patients with cognitive and/or ADL physical functioning disorders and develops appropriate plan of care following all regulatory and clinical practice standards.
  • Utilizes standardized assessments and evidence-based practice to support clinical interventions.
  • Provides rehabilitative, skilled and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
  • Administers various procedures as part of the rehabilitation plan including but not limited to: ADL retraining; therapeutic activities; therapeutic exercises; modalities; use of supportive and assistive devices; etc.
  • Assesses patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
  • Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
  • Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
  • Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
  • Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
Qualifications:
 Occupational Therapy license is required.

About the Opportunity:
Ensign Services, Inc. (ESI) seeks an energetic and dynamic Therapy Resource with at least five years’ experience in the skilled nursing facility environment to be part of its San Antonio, TX area operations. This position can be held by a Physical, Occupational or Speech therapist and offers a unique opportunity to support our in-house therapy programs in this region of Texas.

Experience/Qualifications: 
Licensed Therapist 
A minimum of five years’ SNF experience 
Knowledge of PDPM coding rules and regulations
Understanding of the legal and regulatory framework governing the skilled nursing industry
Ability to work with all levels of management on all aspects of therapy operations in a skilled nursing setting
 
Duties:  
  • Works closely with Directors of Rehabilitation and other facility leadership to ensure excellence in delivery of therapy services.
  • Supports the facility team in whatever ways possible.
  • Develops and maintains Ensign culture with all members of the therapy team.
  • Identifies areas of potential improvement and creates and implements training to accomplish this goal
  • Ensures adherence to state and federal regulations pertaining to our industry
  • Travel required 90% of the time, up to 150 miles
  • Candidate location in the greater Houston/East, TX area is strongly encouraged
