About You:
- You are a Licensed Physical Therapist and can work with a diverse patient population at all experience levels.
- You’re passionate about making a positive impact on patients’ lives and helping them regain independence.
- You’re committed to delivering evidence-based treatments and collaborating with an interdisciplinary team.
- Assess, evaluate, and develop treatment plans for patients with cognitive and/or ADL physical functioning disorders.
- Administer rehabilitation interventions including ADL retraining, therapeutic exercises, modalities, and the use of assistive devices.
- Regularly assess patient progress, adjusting treatments accordingly, and communicate updates with the healthcare team.
- Provide counseling and consultation to patients, families, caregivers, and service providers.
- Document treatment plans and outcomes in line with regulatory and clinical practice standards.
- Contribute to patient-centered care by evaluating treatment effectiveness and making necessary referrals.
