THE GARRISON CARE CENTER in Lubbock, TX, is a 120-bed skilled nursing facility with strong ties to Texas Tech Health Sciences Center and the Lubbock community. We are currently seeking an innovative PT, OT, PTA, COTA or SLP to lead our in-house therapy team as Director of Rehabilitation. You will create and implement new rehabilitation programs for our deserving patients and manage the daily operations associated with a busy in-house therapy department.

Duties:
  • Implements and monitors the provision of facility Rehab policy and procedures
  • Supervises rehab staff and ensures effective rehab staff orientation, training and ongoing competency
  • Coordinates rehab admissions and distribution of caseload
  • Assists in the case management/tracking of Medicare A rehab cases under the PDPM system
  • Recruits rehab staff
  • Manages and analyzes department metrics including accurate billing
  • Ensures that rehab department is compliant with all regulatory issues. This includes assurance that department records are completed and maintained, such as medical record documentation, reimbursement denials management and other survey compliance requirements.
  • Maintains relationships with hospital therapists, physicians and other key community members
  • Provides direct patient treatment
Qualifications:
Licensed PT / OT / PTA / COTA or SLP
At least 3 years of experience as a therapist
Supervisory and/or administrative experience in long term care rehabilitation preferred
Knowledge of Medicare PDPM and ability to manage a variety of payors effectively
Strong organizational and leadership skills

The Garrison Care Center in Lubbock is a 120-bed skilled nursing facility with strong ties to Texas Tech Health Sciences Center and the Lubbock community. We are seeking a Full Time Outpatient Speech-Language Pathologist (SLP) to join our therapy team. As part of this amazing team of clinicians, you will have opportunities for training and continuing education in Think Thin/dysphagia, Abilities Care and dementia, stroke and neuro rehabilitation, and respiratory recovery programs.
Duties:
• Screen and evaluate patients with communication, cognitive, or swallowing disorders, developing appropriate care plans following regulatory and clinical standards.
• Deliver rehabilitative treatment to patients with communication, cognitive, and/or swallowing disorders, ensuring compliance with regulatory and clinical practice requirements.
• Evaluate treatment outcomes, modifying services based on patient evaluation and making referrals as needed.
• Provide consultation and counseling to patients, families, caregivers, and other service providers related to speech disorders.
• Generate comprehensive discharge summaries in adherence to regulatory and clinical requirements.

Qualifications:
Speech-Language Pathology license is required.

Experience/Qualifications:

  • Licensed Occupational Therapist (Required)
  • Open to OTs of all experience levels

Key Responsibilities:

  • Effectively screen and evaluate patients with cognitive and ADL physical functioning disorders, creating individualized treatment plans that meet regulatory and clinical practice standards.
  • Utilize evidence-based practice and standardized assessments to support clinical interventions.
  • Provide rehabilitative, skilled treatment interventions including ADL retraining, therapeutic activities, and assistive device training.
  • Assess patient progress and adjust treatments as necessary to meet goals.
  • Work closely with families, caregivers, and the healthcare team to ensure comprehensive patient care.
  • Communicate progress effectively with the healthcare team and document outcomes according to regulatory standards.
  • Provide discharge summaries and recommendations for continued care.
