Key Responsibilities

  • Screen and evaluate patients with cognitive and ADL functioning disorders; develop tailored plans of care.
  • Utilize standardized assessments and evidence-based practices to inform clinical interventions.
  • Provide rehabilitative treatments while adhering to regulatory standards.
  • Administer rehabilitation procedures, including ADL retraining and therapeutic modalities.
  • Assess patient progress and modify treatment plans as needed; document accordingly.
  • Evaluate treatment outcomes and adjust services based on patient feedback, making referrals as necessary.
  • Consult with patients, families, and caregivers regarding physical disorders.
  • Communicate effectively with supervisors and the healthcare team about patient progress.
  • Prepare comprehensive discharge summaries in compliance with regulatory requirements.
