Pacific Care and Rehabilitation is located in Hoquiam, WA, a beautiful Olympic Peninsula location on the Pacific Coast of Washington State, we have a beautiful 3,000 square foot rehab facility with an amazing gym, new therapy equipment, excellent ADL space, therapist offices, and room to grow!
 
Duties:
  • Provide rehabilitative, skilled, and medically necessary treatment interventions to patients with cognitive and/or physical functioning disorders, following regulatory and clinical standards under Occupational Therapist supervision.
  • Administer various rehabilitation procedures, including ADL retraining, therapeutic activities, exercises, modalities, and the use of supportive and assistive devices, under Occupational Therapist supervision.
  • Assess patient progress and communicate with the supervising Occupational Therapist to adjust treatments in accordance with regulatory and clinical practice requirements.
  • Report treatment outcomes’ effectiveness to the supervising Occupational Therapist.
Qualifications:
Graduate of an accredited Occupational Therapy Assistant program.
Hold a current and active State license/registration where applicable.

Our Colorado market is excited to announce our growth in the Northern Colorado area, and we are currently seeking a Full Time SLP to join our crew of in-house therapists at McIntosh Care & Rehabilitation Center in Longmont, CO. Help us expand therapy programming to include Abilities Care (cognition), ACP programs, Big and Loud and wound care. As a therapy professional, you will have the freedom to create and implement additional programs to best meet the needs of the patients in this facility. 
 
Our therapy team expansion will be a strong focus, and you will receive strong orientation, training and on-going support as we grow!

Qualifications:
Licensed Speech-Language Pathologist
Must hold current ASHA Certificate of Clinical Competence or be qualified for Clinical Fellowship Year

Key Responsibilities

  • Effectively screens/evaluates patients with cognitive and/or ADL physical functioning disorders and develops appropriate plan of care following all regulatory and clinical practice standards.
  • Utilizes standardized assessments and evidence-based practice to support clinical interventions.
  • Provides rehabilitative, skilled and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
  • Administers various procedures as part of the rehabilitation plan including but not limited to: ADL retraining; therapeutic activities; therapeutic exercises; modalities; use of supportive and assistive devices; etc.
  • Assesses patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
  • Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
  • Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
  • Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
  • Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
