  • Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
  • Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
  • Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
  • Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.
Qualifications:
Physical Therapy license is required.

About the Role
  • POST Rehabilitation and Wellness partners with an organization that supports individuals with special needs through a structured day program and group home settings. Therapy services are primarily provided at one central day program location, with additional opportunities to provide services in nearby group homes.
  • The day program operates Monday–Friday (occasional weekends as needed)
  • Residents are transported from their group homes (approximately 10 minutes away)
  • There are 10 group homes, all within 10 minutes of each other
  • Patients remain on caseload long-term while residing in the group home
  • All therapy services provided are maintenance therapy
Schedule & Flexibility
  • Patients are typically seen onsite at the day program during weekday hours
  • PRN role with consistent and predictable opportunities for hours
  • Hours starting at 10-20 hours per week with increase up to 40 hours per week, if desired
