Duties:
• Screen and evaluate patients with communication, cognitive, or swallowing disorders, developing appropriate care plans following regulatory and clinical standards.
• Deliver rehabilitative treatment to patients with communication, cognitive, and/or swallowing disorders, ensuring compliance with regulatory and clinical practice requirements.
• Evaluate treatment outcomes, modifying services based on patient evaluation and making referrals as needed.
• Provide consultation and counseling to patients, families, caregivers, and other service providers related to speech disorders.
• Generate comprehensive discharge summaries in adherence to regulatory and clinical requirements.

Qualifications:
Speech-Language Pathology license is required.

We are seeking someone who is passionate about their profession and takes pride in the work they do. As Outpatient Coordinator you will be responsible for building and strengthening relationships with current and prospective outpatient referral sources by completing face-to-face meetings with physicians, office managers, referral coordinators, and individuals in the community. You will educate these individuals on the services our facility offers, discuss patient care, referrals, and thank them for existing referrals. As an Evaluating Therapist, you will be treating older adults in our skilled nursing facility as well as in the community. This position offers a flexible schedule of 3 days/week; a full-time schedule is available if requested.

Responsibilities:
  • Build and strengthen relationships with current and prospective referral sources by completing face-to-face meetings with physicians, office managers, referral coordinators, and individuals in the community.
  • Identify and market to local primary and specialty physicians to address their patient’s outpatient therapy needs.
  • Assist the Director of Rehab with coordinating community events, public relations campaigns, or specialty marketing campaigns
  • Manage outpatient therapy schedule to establish and maintain POC as well as coordinate with other therapists to reschedule any missed visits.
  • Provide therapy services to long-term care, skilled in-patient, and outpatients within the facility.
  • Provide therapy services in the outpatients’ homes, group home, ALF/ILF
  • Report back to DOR after each day with updates
Requirements:
  • Licensed Physical Therapist or Physical Therapist Assistant
  • Experience in marketing in healthcare is optional, but preferred
  • Exceptional interpersonal skills, including the ability to quickly build rapport
  • Outgoing and personable
  • Willingness to become involved in local community events
  • Minimum of 1 year experience in SNF and/or home health

Oak View Health & Rehabilitation in Conway, SC is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery.  We are currently seeking a part time Physical Therapist for our in-house rehab program!
 
Duties:
  •  Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
  •  Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
  •  Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
  •  Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
  •  Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.
 
Qualifications:
 Physical Therapy license is required.
 Open to Physical Therapists at all experience levels. New Grads are welcome!
