Key Responsibilities

Effectively screens/evaluates patients with cognitive and/or ADL physical functioning disorders and develops appropriate plan of care following all regulatory and clinical practice standards.
Utilizes standardized assessments and evidence-based practice to support clinical interventions.
Provides rehabilitative, skilled and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.

• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.

Community-Based Outpatient Physical Therapist (Full-Time) – ALF & Clinic Setting – Montecito Outpatient Rehabilitation – Mesa, AZ

Montecito Outpatient Rehabilitation in Mesa, AZ, is seeking a Full-Time Physical Therapist to join our dedicated in-house therapy team. This community-based outpatient position offers the opportunity to work in both clinic and Assisted Living Facility (ALF) settings. This role will require travel between clinic locations while providing patient-centered care focused on improving quality of life through personalized, hands-on physical therapy. Key Responsibilities: Community-Based Outpatient Care:
Provide individualized physical therapy in both clinic and ALF settings, focusing on improving patients’ physical functioning, mobility, and overall well-being.

Assessment & Evaluation:
Evaluate patients with functional deficits such as weakness, pain, balance, and gait impairments. Thrive in helping and assessing a diverse range of orthopedic and neurologic diagnoses. Our in-clinic team is highly skilled in working with amputees from the community to significantly improve their function and quality of life.

Treatment Planning & Delivery:
Therapeutic exercise to improve strength, balance, mobility, and endurance. Gait training and functional mobility training. Neuromuscular re-education and postural training. Pain management and movement retraining. Assistive device assessment and training. Fall risk assessment and prevention strategies.

Progress Monitoring:
Regularly assess and track patient progress, making necessary adjustments to treatments to ensure optimal outcomes.

Patient, Family & Caregiver Support:
Provide consultations and education to patients, their families, caregivers, and other service providers to ensure a well-rounded approach to rehabilitation.

Collaboration:
Communicate effectively with supervisors and interdisciplinary team members regarding patient progress, barriers to treatment, and care plans.

Discharge Planning:
Ensure timely and comprehensive discharge summaries are provided in line with regulatory and clinical requirements.
