Phoenix Mountain Therapy is seeking a passionate Physical Therapist to join our growing outpatient therapy team serving Assisted Living and Independent Living communities throughout the Chandler area. What You’ll Do

  • Evaluate and treat patients with orthopedic, neurological, balance, gait, mobility, and functional impairments
  • Develop individualized treatment plans using evidence-based practice
  • Help patients improve strength, mobility, balance, endurance, safety, and overall functional independence
  • Collaborate with physicians, community staff, families, and interdisciplinary team members
  • Educate patients and caregivers on treatment plans and home exercise programs
  • Complete timely and accurate documentation

Qualifications

  • Current Arizona Physical Therapist license (or eligibility for licensure)
  • Strong communication and interpersonal skills
  • Passion for serving the geriatric population

Chandler Post Acute and Rehabilitation in Chandler, AZ, is seeking a Full Time Occupational Therapy Assistant to join our in-house therapy team.  As a member of our team, you can expect excellent leadership, facility level decision making, competitive compensation and most of all, a fun work environment where you are appreciated and supported to do your best work.

Duties:
  • Provide rehabilitative, skilled, and medically necessary treatment interventions to patients with cognitive and/or physical functioning disorders, following regulatory and clinical standards under Occupational Therapist supervision.
  • Administer various rehabilitation procedures, including ADL retraining, therapeutic activities, exercises, modalities, and the use of supportive and assistive devices, under Occupational Therapist supervision.
  • Assess patient progress and communicate with the supervising Occupational Therapist to adjust treatments in accordance with regulatory and clinical practice requirements.
  • Report treatment outcomes’ effectiveness to the supervising Occupational Therapist.
Qualifications:
Graduate of an accredited Occupational Therapy Assistant program
Hold a current and active State license/registration where applicable
Candidates with all levels of experience are welcome
